Security fix: hardened the WordPress login form’s administrator OTP-bypass flow so it always requires a verified password or OTP before issuing a session, in every “Login with only OTP” configuration.

Security fix: the OTP Spam Prevention addon no longer trusts client-supplied IP headers (X-Forwarded-For, CF-Connecting-IP, etc.) for rate-limiting; the real connection IP is used unless a trusted reverse proxy is explicitly configured.
